Our emergency department at Lake Shore Health Care Center offers a dedicated team of physicians, nurses, and technicians that are ready to help you 24-hours a day.

 

Medical/ Surgical Care
 

With our team of dedicated physicians, surgeons, nurses and technicians, TLC Health offers high quality medical and surgery services.

 

Primary Care 

 

From routine check-ups to treatment of an illness or chronic medical condition, TLC Health Network’s primary care centers, conveniently located in Gowanda, Forestville, and Conewango Valley, are designed to meet the needs of children, adults, and senior citizens.

 

Behavioral Health 

 

Behavioral health services at TLC Health Network are designed to effectively meet the needs of community residents with emotional or psychiatric problems. Our inpatient program provides treatment in a modern, confidential, and comfortable setting.

 

TLC Health’s Assertive Community Treatment (ACT) program provides care for patients with serious and persistant behavioral health issues.

 

Chemical Dependency 

 

TLC's chemical dependency program is designed to guide our clients toward a new life free of drugs and alcohol. Outpatient clinics are currently located in Derby and Cassadaga.  Limited transportation assistance options may be available with advance notice.

 

TLC also offers a long term program called IRRS (Intensive Residential Rehabilitation Services) that houses 25 chemically dependent females and fills the critical gap that often exists between inpatient and outpatient chemical dependency services.

 

Long Term Care 

 

Sometimes home care can no longer meet the needs of individuals who have become more dependent. At TLC Health’s skilled nursing home, residents enjoy the benefit of individualized high quality care.

 

Imaging 

 

Using state-of-the-art technology, TLC Health offers digital X-ray, CT scan, bone density exam, nuclear medicine, mammography, and ultrasound services.

 

Rehabilitation 

 

An integral part of the healing process, rehabilitation services at TLC Health hospitals offer the services of physical, occupational and speech therapists, as well as nurses trained in cardiac rehabilitation, to help you continue on your road to recovery.

 

Health and Wellness 

 

TLC Health is committed to improving the health of the communities we serve. Designed to support and promote community health initiatives, TLC Health’s health and wellness program offers programs on topics such as nutrition, cancer, osteoporosis, heart health and exercise.

  

Home Health Care 

 

An alternative to nursing home placement, TLC Health’s home health services provide you with coordinated health services all in the comfort of your own home.

 

Medical Laboratory 

 

TLC Health offers high quality clinical laboratory services with service locations in Gowanda, Irving and Derby.
