Health Information Management (Medical Records)

The Health Information Management (HIM) department is responsible for maintaining all patient medical records at our facilities, including all inpatient, outpatient, and ancillary department records.  Patients and former patients may request transfer of their records to their primary physician or specialists through HIM.  We also handle all releases of medical information.  Please remember that we are required to have each patient or their legal representative sign an authorization to allow us to release confidential medical information.

Birth Certificates

Brooks Memorial Hospital does not have copies of Birth Certificates, but the City of Dunkirk does.  You will need to contact the Dunkirk City Clerk to get an official copy of your birth certificate.  They will charge you $10 for this, but you can do it via mail.  Write a letter to them requesting the certificate and have your signature on the bottom of the letter notarized.  They will also require a photocopy of your driver’s license.
