TLC Health Network serves a large portion of Chautauqua, Cattaraugus, And Erie Counties. So that we may best meet the health care needs of such a large and diverse region, TLC works closely with other area health care providers and county health departments to conduct periodic Community Health Needs Assessments (CHNA). The information received as a result of these assessments is then reviewed by the Board and Administration of TLC and used to implement strategies to improve the overall health and wellness of the population we serve.
